The Member Engagement Specialist (MES) will play a vital role in Full Circle's member strategy and support by working closely with contracted providers, local agencies and directly with members themselves to increase education, awareness and engagement in Enhanced Care Management, Community Supports and Community Health Worker Services through CalAIM. The Member Engagement Specialist will report directly to the Member Engagement Supervisor.
Responsibilities
- Understand the needs of the communities served by Full Circle Health Network
- Develop community based pipelines for referring members into ECM, CS, and CHW services
- Utilize data and reporting to engage with and connect members to appropriate providers within Full Circle's provider network
- Provide virtual and in-person support to providers to help build out internal referral pipelines and processes to increase member engagement
- Use provider portals and HIEs to conduct research and connect members with the Full Circle Health Network
- Develop relationships with providers and local agencies/organizations working with youth and families to increase referrals
- Process and complete the handoff of Community Pathway Referrals
- Monitor client assignment and support the engagement timeframes established in Full Circle P&Ps
- Support and engage members who are interested in enrolling in ECM, CS, or CHW services
- Escalate issues appropriately to the Engagement Team Supervisor and provide input into continuous quality improvement efforts
- Manage closed loop referrals
- Complete all documentation timely and accurately
- Attend meetings with managed care plans and providers as assigned
- Attend and complete all required training on a regular basis
- Support data entry of services as needed into health plan portals
- Ensure client patient confidentiality
- Adhere to Company Code of Conduct
- Perform all other duties as assigned
Required Qualifications
- CHW license or Associate's Degree required
- At least 3 years of relevant work experience
- Member engagement or direct work with community members: 2 years
- Highly organized with the ability to multi-task and work independently or within a team to meet deadlines
- Ability to work effectively in a remote team using technology
- Computer competent- Microsoft suite specifically
- Experience with data analytics
- Reliable transportation and the ability to meet with providers in person and attend events locally as needed.
- Organizational skills, ability to analyze and synthesize information efficiently and thoroughly.
- Exceptional communication skills.
Preferred Qualifications
- Prior experience as a Community Health Worker, Peer Support Specialist, Lead Care Manager or similar role.
- Experience working with CBOs or other health care/behavioral health care providers.
- Highly organized with the ability to multitask
- Ability to work both independently and with a team to meet deadlines
- Multilingual capabilities.
