Job Title: Certified Family Partner (CFP) - Youth Crisis Outreach Team

Location: Brazoria and Galveston Counties

FLSA Status: Non-Exempt

Working Status: Full Time with Benefits

Summary:

The Certified Family Partner (CFP) is a vital member of the Youth Crisis Outreach Team, providing peer-based family support, advocacy, and engagement to youth experiencing behavioral health crises and their caregivers. Drawing from lived experience as a caregiver of a child with behavioral health needs, the CFP supports families during high-stress situations, helps them navigate systems of care, and promotes family voice, choice, and resilience throughout the crisis response and stabilization process.

The Certified Family Partner (CFP) ? Youth Crisis Outreach Team will be responsible for the following:

Essential Job Duties:

• Provide peer support, education, and emotional support to caregivers and families of youth experiencing behavioral health crises
• Engage families during crisis outreach, assessments, and follow-up services in homes, schools, hospitals, shelters, or community settings
• Share lived experience appropriately to build trust, normalize challenges, and instill hope
• Assist caregivers in understanding crisis response processes, safety planning, and available community resources
• Advocate for family voice and youth-centered, family-driven care within the multidisciplinary crisis team
• Support families in navigating behavioral health, child-serving, educational, and social service systems
• Collaborate closely with clinicians, crisis counselors, and other team members to support coordinated care
• Document services and family contacts accurately and timely in accordance with program and regulatory requirements
• Participate in team meetings, trainings, supervision, and quality improvement activities
• Maintain professional boundaries, confidentiality, and ethical peer support practices at all times
• Will be responsible and accountable for the adherence to and promotion of the agency compliance policy and will require communication with various departments to foster, develop and continue our community partnerships as it relates to the position.
• Treat all individuals with the utmost courtesy, decency and respect.
• Provide responsive, and efficient services.
• Interact with staff, consumers, local entities and advocacy groups to provide a service that meets local needs.
• Provide highest level of Customer Service for both internal and external customers.
• Required to report personnel leave; flex time and emergency leave to assigned supervisor.
• Strict adherence to the professional, ethical, and legal requirements set forth in GCC's Business Code of Conduct as well as the Corporate Compliance Plan.
• Consistent adherence to the Center's Dress Code.
• Consistent adherence to Center safety guidelines and expectations.
• May be required to travel within Brazoria and Galveston Counties.
• Out of region travel may be required to attend trainings.
• Must maintain a clear driving record and driver's license in accordance with our Driver Policy.
• Requires current Texas Driver's License, reliable transportation and must maintain current auto liability insurance.

• Certification as a Certified Family Partner (or eligibility to obtain certification within one year)

• Lived experience as a parent, caregiver, or legal guardian of a child or youth with a behavioral health, or emotional diagnosis
• Knowledge of youth behavioral health systems, crisis services, and family support principles

• Previous experience working in crisis services, mobile outreach, or community-based behavioral health programs
• Experience working with diverse populations and understanding of cultural humility
• Knowledge of trauma-informed care, recovery-oriented systems, and family-driven care models
• Bilingual or multilingual skills

• Effective interpersonal skills
• Ability to accept responsibility and exercise independent judgment in making decisions
• Capable of recognizing necessary response to unexpected situations
• General knowledge of mental health disorders
• Self motivated with good time management skills
• Mature problem solving approach to daily work
• Ability to work cooperatively with immediate work group and individuals external to the program

• Work will be performed in the Office, Field and/or Remotely setting(s).
• May require varied work hours depending on need of clients.
• Out of region travel may be required to attend trainings.
• May require travel to various locations within Brazoria and Galveston Counties. Requires current Texas Driver's License, reliable transportation safe and current with all state requirements, and liability insurance.

• Assist supervisor when requested to complete special projects and complete all assignments within a time frame negotiated with the supervisor. Completion of other duties as assigned by supervisor as it relates to the program and position.
